Social Security Number

J-1 visa trainees may apply for a Social Security Card using the following procedure:

1. Complete an SS-5 form.

2. Submit your application to the Social Security representative at the Hendry County Courthouse at 9:30 AM on the first Tuesday of every month. Bring the following documents with you to the Courthouse:

  * completed and signed SS-5, employment verification form (your visa sponsor contract),
  * original passport
  * original DS-2019 and
  * original I-94.

3. If your application is approved, your Social Security Card should arrive in 10-15 days.
